Mail Order Form Instructions
		To order a book or video, simply follow these instructions: Download this form: Video Order Form The order form is in Excel spreadsheet format. You may either enter the order information in Excel, or print the form and write it in. Enter the following information: "Ship To" Address Quantity and Extended Prices for each product Subtotal IL residents only - add 7.75% tax (mutiply Subtotal by 0.0775) Shipping (see table at bottom of order form) Total Remittance Make check or money-order payable to "Center for Taiji Studies" For orders outside the United States, send international money order or bank draft payable in U.S. dollars. Mail your order plus payment, and mailing address to: Center for Taiji Studies P.O. Box 11386 Champaign, IL 61826 For more information about the videos, call 217-351-9388, or contact us. Please note that a money order will not suffer the delay of clearing the bank, and will therefore ship sooner than a personal check. For even faster service, order through our Online Order Page. All sales are final. Defective tapes will be exchanged upon return of the tape, accompanied by the invoice, within 30 days of receipt.
